• Provide Smart Apartment Solutions with a current resume and Summary of Qualifications.
• An authorization for background screening. We do require a background check on all Smart candidates. A client may require additional screenings including credit, criminal, previous employment, references, etc. Additional testing may be requested such as drug testing, personality, skills or aptitude testing. It will be your decision if you would like to accept or decline any required tests.

* Background Release

Smart Apartment Solutions, LLC performs background and reference verifications on candidates that they represent and present to client companies. To move forward in this process you must agree to the following terms by thoroughly reading the following authorization. I hereby authorize Smart Apartment Solutions to conduct a reference and background investigation that will include, but not limited to, arrest and/or criminal records, credit history, driver/motor vehicle abstract, employment verification, education verification and social security number verification. I also understand that the information below regarding date of birth is requested for the sole purpose of gathering the above information correctly, and will not be used to discriminate against me in violation of any law. I hereby authorize without reservation, any party or agency to furnish the above-mentioned information. I fully understand that the results of this background investigation and the conclusions drawn therefore from Smart Apartment Solutions, LLC, its officers, agents and employees may prove unfavorable to me. I understand that disclosure of a felony criminal record will not automatically disqualify me from employment consideration, but I do, however, understand that falsification of information on my history may bring about immediate dismissal from an employer, or refusal to represent me for direct hire. I do nonetheless hold Smart Apartment Solutions, its officers, agents, employees and any of my former employer’s officers, agents and employees free and harmless from any claim I might otherwise have against them for any damages or liability to me resulting from this background investigation.

* Confidentiality Agreement

It is understood and agreed to that the below identified discloser of confidential information may provide certain information that is and must be kept confidential. To ensure the protection of such information, and to preserve any confidentiality necessary under patent and/or trade secret laws or intellectual property, it is agreed that the Confidential Information to be disclosed can be described as and includes:

  1. Invention description(s), technical and business information relating to proprietary ideas and inventions, ideas, patentable ideas, trade secrets, drawings and/or illustrations, patent searches, existing and/or contemplated products and services, research and development, production, costs, profit and margin information, finances and financial projections, customers, clients, marketing, and current or future business plans and models, regardless of whether such information is designated as “Confidential Information” at the time of its disclosure.
  1. This Agreement imposes no obligation upon Recipient with respect to any Confidential Information (a) that was in Recipient’s possession before receipt from Discloser; (b) is or becomes a matter of public knowledge through no fault of Recipient; (c) is rightfully received by Recipient from a third party not owing a duty of confidentiality to the Discloser; (d) is disclosed without a duty of confidentiality to a third party by, or with the authorization of, Discloser; or (e) is independently derived by Recipient.
  1. Parties agree that the subcontractor and or candidate for direct hire, for a period of one year, shall not discuss, pursue, consult or accept a position as a direct hire with a client that they have previously been assigned to or presented by Smart Apartment Solutions, in any capacity. Smart Apartment Solutions must initiate any discussions regarding a direct hire position and subcontractor and or direct hire candidate agrees to such representation.
  1. This Agreement states the entire agreement between the parties concerning the disclosure of Confidential Information. Any addition or modification to this Agreement must be made in writing and signed by the parties.
  1. If any of the provisions of this Agreement are found to be unenforceable, the remainder shall be enforced as fully as possible and the unenforceable provision(s) shall be deemed modified to the limited extent required to permit enforcement of the Agreement as a whole.

WHEREFORE, the parties acknowledge that they have read and understand this Agreement and voluntarily accept the duties and obligations set forth herein.

* Verification Of Past Employment

Smart Apartment Solutions may need to verify past dates of employment, title and wage. Submitting this form is your authorization to conduct necessary verifications for staffing and permanent placement.

* Independent Contractor Status

I am a Sole Proprietor. I am performing work on this assignment as an Independent

Contractor, for workers` compensation purposes, and therefore, I am not entitled to workers` compensation benefits under the provisions of the workers` compensation laws as provided by the Department of Labor. I understand that I must complete a Waiver of Workers’ Compensation every year, and failing to do so, could result in termination of my status with Smart Apartment Solutions.

In addition to my Independent Contractor status, I understand that I will be responsible for with holding my own taxes and will receive a 1099 at the beginning of the year. To complete my registration I understand that I must complete a W-9 provided by Smart Apartment Solutions.

* Premium Pay Policy

  1. It shall be the policy of Smart Apartment Solutions to confirm the availability, and acceptance, of an assignment for each Smart Staffer weekly.
  2. It is the expectation that the Smart Staffer will fulfil the confirmed assignment in it’s entirety. If for any reason, the Smart Staffer does not fulfill the entire assignment, and it is deemed by Smart Apartment Solutions to be an unexcused absence, the Smart Staffer agrees that this unexcused absence will affect the premium pay for the week in question, reducing the premium pay by $10.00 per day, for the five days of that affected week.

* Direct Deposit

Smart Apartment Solutions does not issue checks, therefore I agree to complete the Direct Deposit form provided by Smart within my first week of my first assignment.
